Remove "hibernate3" library from project's library
Fix missing Hibernate library at Hibernate facet configuration
In Dependencies tab of the module will show 2 "hibernate3" items.
Note: I picked Hibernate's JARs from local disk. So I'm not sure it will happen if we download from Maven repository or not.

Reproduce:
Create a project with Hibernate facet
Remove "hibernate3" library from project's library
Fix missing Hibernate library at Hibernate facet configuration
